As of September 2026, the FDA and the EMA have not approved Semax. The record says it is approved only in Russia, for stroke and cognitive uses. Nearly all the human data are older, small Russian studies. Outside Russia it circulates informally as a research chemical, and you cannot tell by looking what one of those vials holds.

What is Semax?
Semax is a lab-made chain of seven amino acids, based on a fragment of a larger molecule called ACTH. Russian researchers at the Institute of Molecular Genetics, Russian Academy of Sciences, first described it in 1991. It is registered as a drug in Russia, and outside Russia the record says it circulates informally as a research chemical.
Does Semax work?
Two Russian trials in stroke reported benefit. The record calls the human trials small, with limits in their methods, and says no large trial to Western standards has repeated them.
What we found. 2 completed randomized trials counted on the record. The 2 human studies listed are both Russian and both in stroke. 1 animal study, in rats.
| Study | Tested in | Status | What it shows |
|---|---|---|---|
| Semax in patients at different stages of ischemic stroke | People | Published in 2018. A Russian trial | Reported improved outcomes across stroke stages, in a small Russian group |
| Semax in the acute period of hemispheric ischemic stroke | People | Published in the 1999-2001 era | An early Russian trial that reported benefit in acute stroke. The record's note reads "limited blinding/rigor described" |
| Semax during experimental cerebral ischemia | Animals | Published | In a rat model of ischemia, Semax reduced neurological deficit and the memory loss that came with the injury |
A result in rats is a lead to follow up. It cannot stand in for a result in people.
How we count a trialIs Semax safe?
On this record, nobody knows. It holds no human safety finding for Semax, and it says no large-scale Western human safety trial exists. The human trials it lists are small and report on stroke outcomes. The rat study looked at benefit, with no safety result on the record. That covers the compound. The product sold outside Russia raises a second question, taken up below.
Is Semax legal?
- United States
- Not approved by the FDA. The record says it is not sold as a medicine in the US.
- European Union
- No EMA marketing authorisation. The record says it is not sold as a medicine in the EU.
- Elsewhere
- The record says it is approved only in Russia, for stroke and cognitive uses. One field lists ischemic stroke, transient ischemic attack, and cognitive and memory disorders.
- Sport
- Not named on WADA's list. That is not clearance.
Not medical advice. Approval and sport status as of . Medical disclaimer
What is in a vial sold as Semax?
Outside Russia, the record says Semax circulates informally as a research chemical, marketed for focus and for protecting nerve cells. It says the marketing leans on Russian animal studies and a limited number of small Russian stroke trials. A "research use only" label guarantees nothing about purity. A certificate of analysis counts only if it names your lot number, a lab with no stake in the result, and the method used. Purity and identity are separate tests.
Three questions to ask the seller:
- Which lot number is this certificate for?
- Who ran the test, and are they independent of you?
- Does the report confirm identity by mass spectrometry, or purity alone?
What don't we know?
- Whether the stroke findings hold up. No large trial to Western standards has repeated them.
- What its risks are in people. The record says no large-scale Western human safety trial exists.
- Whether it does anything for focus. The human trials on this record were in stroke.
- What is in any given vial.
The other side
Semax is approved in Russia, with stroke trials on the record. A personal result is real to the person who had it. It cannot separate the compound from the vial, or from an ordinary good week.
What would change this record?
- A large randomized trial with a placebo group, run independently.
- Its publication in a peer-reviewed, internationally indexed journal, outside Russia's own trial system.
Three questions to ask next
- What does 'research use only' actually mean?
- How can you tell if a peptide vendor is legitimate?
- How do you read a certificate of analysis?
Ask better questions.
More questions about Semax
Is Semax approved by the FDA?
- No. The FDA has not approved Semax, and it has no EMA marketing authorisation. The record says it is not sold as a medicine in the US or the EU.
Is Semax approved anywhere?
- In Russia, the record says, for stroke and cognitive uses. It calls that the only approval. The record holds no official document behind the line.
Has Semax been tested in people?
- Yes, in two Russian stroke trials on the record. Both reported benefit. The record describes the human trials as small, with limits in their methods.
Is Semax the same as Selank?
- No. Semax is based on a fragment of ACTH and is approved in Russia for stroke and cognitive uses. Selank is modeled on tuftsin and was developed for anxiety. The two records name the same Russian institute.
Is Semax banned in sport?
- It is not named on WADA's list. The record gives no sport source for that. If you compete, check the current List.
